About the Opportunity

MDS is seeking to employ a Permanent Support Facilitator for our Partners in Recovery program. This role supports people experiencing severe and persistent mental illness through the coordination of local health, clinic, community and social services.

Key Responsibilities for this position include:

  • Provide coordination of supports for referred consumers
  • Commitment to principles and practices of recovery in all dealings with consumers, carers and stakeholders
  • Undertake initial assessments of consumer's service and support needs
  • Support consumers to transition to the NDIS
  • Develop, monitor and regularly review the consumer's Partners in Recovery Action Plan

The successful applicant will have:

  • Appropriate qualifications in a relevant discipline (eg. social work, social welfare, human services) at a Certificate IV or higher and/or demonstrated extensive experience
  • Knowledge of NDIS in the context of supporting individuals with a psychosocial disability
  • A thorough understanding of and commitment to recovery-oriented principles in mental healthcare
  • Demonstrated skills and knowledge in working with people to build personal capacity;
  • Current Drivers Licence and access to a reliable motor vehicle

The Position is Permanent Full time, on a flexi-time contract. This role is covered under the SCHCADS Award Level 5 ($37.03-$39.11 per hr). Also offered is salary packaging, flexible working hours, mobile phone and leave loading. A copy of the job description can be found here
