About the Opportunity

MDS is looking for a part time support coordinator to join a busy team to work with caseloads to assist particularly with capacity building of participants and families.  This role would be based out of Campbelltown and work 3 days per week M, W and F, working from 9 am to 4 pm supporting our current growing caseload.  The successful applicant will have a strong knowledge of the NDIS, skilled in working with a complex caseload and have a particular skill in education and support to improving levels of independence in the management of an NDIS plan.  The applicant will have a reliable vehicle and drivers licence so as to be able to move around the area to meet with participants and families as well as be focused on achieving the outputs of the role on a daily basis.

Key Responsibilities for this position include:

  • Provide varying degrees of coordination and transition supports for children, young people and adults with a disability
  • Support implementation and identify options for all supports in the plan, including informal, mainstream and community, as well as funded supports
  • Strengthen and enhance the participant's abilities to coordinate supports and participate in the community, reach decisions and develop agreements with support providers
  • Work within the funding levels allocated for each person and monitor and track billable hours
  • Managing a high volume caseload

The successful applicant will have:

  • Certificate IV or higher in Community Services/Disability Work/Case Management OR relevant demonstrated knowledge and experience
  • Knowledge of the National Disability Insurance Scheme (NDIS) and the disability sector
  • High professionalism and experience working collaboratively with multiple stakeholders in formal and informal partnerships to achieve outcomes.
  • High standard and ability to articulate information, factual and evidence based written reports
  • Ability to meet deadlines and manage competing priorities
  • NSW Drivers Licence
  • NDIS Worker Clearance
  • Working With Children Clearance
  • Current Covid-19 vaccination

Desirable:

  • Previous experience working in a Support Coordination/Case Management/Care Coordination role
  • Previous experience working with psychosocial/mental health support services or personal/lived experience in mental health

This is a permanent part time position working 3 days per week. Working Part Time 9 am to 4 pm  6 days a fortnight. MDS uses a hybrid working model, allowing staff to work remotely up to 2 days per week. This role is covered under the SCHCADS Award Level 4 classification ($41.52 - $44.68 per hour).
